This privacy policy sets out how Ali Murch OT (we/us) handles your personal information and is adopted from the Australian Privacy Principles (APPSs) contained in schedule 1 of the Privacy Act 1988. This requires our practice to have an open and transparent management of personal information, including having a privacy policy.

This privacy policy is intended to provide a general overview of our policies for the handling of your personal information. By receiving services from Ali Murch OT and/or providing your personal information to us, you consent to us handling your personal information in accordance with this privacy policy.

Ali Murch OT is an independent practice under the ownership of Ali Murch, Occupational Therapist. Ali Murch OT will ensure the following

Personal data will be used only for purposes laid out in our intake form: for providing quality occupational therapy and administration purposes, including billing. Disclosure of information to and collection of information from other professionals if the therapist deems necessary to facilitate the best outcomes for clients may occur with consent. Professionals, including: GP, Paediatrician, allied health professionals, teachers, child care staff as listed on the consent form.

  • We won't discuss or give personal data to any company, organisation or individual outside of Ali Murch OT unless provided with consent or we are legally obliged to under a Court Order. All staff and students are required to sign a Privacy and Confidentiality Agreement prior to or on commencement with Ali Murch OT.

  • We'll never sell or trade personal data to third party list brokers or direct marketing companies.

  • We'll do our very best to ensure that personal data is kept safe and secure for as long as your details are in our care.

  • Once our relationship has been concluded, we guarantee that confidential information will be destroyed at an appropriate time in the future.

We inform our clients about our practice’s policies regarding the collection and management of their personal information via:

  • Our Client Intake Form under ‘Release of Confidential Information and Client Consent Form’

  • Verbally if appropriate.

Ali Murch is a Mandatory Notifier and has a legal and moral obligation to notify when there is a risk of child abuse or neglect.

Information at Ali Murch OT is stored securely and only practice staff have access to it. Ali Murch OT takes all reasonable steps to ensure that information collected is accurate, complete and up-to-date. Individuals have the right to access their information on request and correct any personal information believed to be inaccurate. If relevant personal or health information is not provided, in part or in full, to Ali Murch OT it may result in incomplete assessment. This may impact on the services that are provided. In general, quality improvement or clinical audit activities for the purpose of seeking to improve the delivery of a particular treatment or service would be considered a directly related secondary purpose for information use or disclosure therefore specific consent for this use is not required.

We will confirm that your complaint has been received and set out the time frame we require to investigate your complaint.
